Distance support for patients with COPD and heart failure

In Sweden, approximately 200,000 people live with heart failure (HF) and between 500,000 and 700,000 with COPD (chronic obstructive pulmonary disease). These two diseases are the most common causes of hospital care in Skåne for people aged 65 and over. Studies show that half of these care cases could be avoided if the patient is given increased knowledge about their own illness and the help of digital solutions.

The project Distance support for patients with COPD and heart failure is underway within the framework of the regional director’s decision on digital renewal within primary care.

Four heart projects are planned within Heart failure:

  • Individually adapted training for patients who are reached through Support- and treatment platform 1177.
  • Patients who lack computer skills have the opportunity to borrow a tablet with interactive training and diuretic titration linked to a scale in order to be able to follow and manage their heart failure. 70 patients will be followed for eight months and evaluation takes place within the framework of a research project SMART HF.
  • For more computer-friendly patients, a smartphone application is planned for self-monitoring and the possibility of remote monitoring with individually adapted measurement values/symptoms and intervals.
  • In the longer term, form a Knowledge Center for Heart Failure with an examining and coordinated function.

Three sub-projects are planned within COPD:

  • Training material on COPD for primary care staff.
  • Individually adapted training for patients who are reached through Support- and treatment platform 1177.
  • Smartphone application for self-monitoring and possibility of remote monitoring with individually adapted measurement values ​​/ symptoms and intervals.
